#Chronicpain is Muscle pain.
Muscles are the first and logical location and source for somatic and physical complaints. Muscle collect stresses strains wounds and scars - these are the exact site and pathology of pain. Because Muscles cannot "pull," they have an absolute daily requirement for exercise, stretching, pulling, kneading, and most importantly needling.
Needling comes into two major varieties thanks to thin filament needles a.k.a. "acupuncture needles" C Chan Gunn M.D. and hypodermic intramuscular needling as per Janet G Travell M.D.
It would behoove a scientific society to completely clarify this word "acupuncture" because the word carries dangerous misconceptions, biased, irrational, prejudiced, even bigotry. In reality, the FDA has determined acupuncture is still experimental and investigational after 6000 years.
Muscles neglect will began to falter, atrophy, weaken, and fail to support a quality of life = Muscle failure.
Myofascial pain and dysfunctions MFP&D treatments are within Physical Medicine, and Rehabilitation was perfected back in the 60s.
Surgery and drugs were a 1960s for-profit trick to betray PM&R. The CNS, brain, nerve or skeletal cannot be the primary location of pain. NO one checked or corrected over the 60s now were confusion, pain, and suicides as a result.

The first group did not get acu at the wrist, just at the ankle of the opposite side.
O day and night, but this is wondrous strange!
And therefore give it welcome.
For there are more things in heaven and earth,
Than are dreamt of in your western science.

It's great you're publishing reports on how useful and effective acupuncture is. The central conclusion of the article - that acupuncture works in carpal tunnel by regulating brain and nerve function - is so erroneous as to laughable. Are the researchers saying that carpal tunnel occurs in the brain? When a patient has decompressive surgery is this working by regulating brain function?! Of course not. Carpal tunnel syndrome is understood very clearly as a. compressive swelling of the carpal tunnel in the wrist - hence its name. The only plausible way by which acupuncture can work is by reducing the pathological swelling in the carpal tunnel. When it does this, of course, the brain and nerves then register that things are better.
It would be like a fire brigade putting out a fire for researchers to announce that you can put fires out by turning off fire alarms.

You've presented an interesting conundrum, and I was given the mission 20 years ago to solve it.
Think muscles and think physical therapy.
Throw out most all of what you've read because it is devoid of most of the anatomy, physiology, and pathology of muscle diseases and all of the physical therapy options which can cure muscle derive pain.
There is no such thing as carpal tunnel syndrome, slipped disc, sciatic, frozen shoulder or any other radiologic or virtual terms that doctors use today.
The pain has to come from someplace and the most likely of the soft, connective and muscle tissues.
You have to throw out a lot of what you know because it is not correct, accurate, precise and not grounded in the natural laws of human biology.
The muscle system contains 700 engines which work by contraction.
Muscles collect stresses, tears, wounds and scars as they contract their little hearts out. They will ball up locking out blood flow and adequate healing.
We are required to pull, stretch, knead, unwind all of our muscles on a daily basis, or they will lock up and then try to kill us because we are not obeying the law of our body.
60 years ago of doctors decided that muscles. This ushered in the beginning of "bone cannibalism."
The skeleton and the nervous system along with the brain are pretty much automatic. Yes, we have fallen victim to human vices.

So the abstract states:
As these primary somatosensory cortex subregions are distinctly targeted by local versus distal acupuncture electrostimulation, acupuncture at local versus distal sites may improve median nerve function at the wrist by somatotopically distinct neuroplasticity in the primary somatosensory cortex following therapy.
So it decreases the person's PERCEPTION of symptoms. What about the actual COMPRESSION of the nerve that causes the symptoms? It is that compression of the nerve which causes symptoms. If I have a fifth of scotch I won't notice it either, but I'll still have carpal tunnel and eventually permanent nerve damage.
Unfortunate information to just spread around, as acupuncture does not cure or improve carpal tunnel, but only decreases the brain's perception of symptoms, and so allows continued nerve compression which can lead to permanent damage.

I've studied "acupuncture," Dry needling, wet needling, bio-puncture along with a lot of other puncture type of treatments and determined They all work perfectly as long as you obey the laws of human biology. Acupuncture is being marginalized, prejudiced, biased and there are bigots and haters do not want this modality to be standardized.
In reality, there were a group of physicians had cracked this "pain in the body code" 60 years ago. Read up on Janet G Travell, David Simons, Edward Rachlin, Stewart Hackett, C Chan Gunn, Joseph Helms and many others.
The needle is simply a key which unlocks mother nature. Stimulate, agitate, stir, reboot, break up stretch it out and then mother nature can do her work more thoroughly.
The basic biology is injury, intramuscular strains in wounds and scars + stretch, kneading, pushing and pulling + intramuscular needling = cure.
This is no laughing matter.
22 veterans will kill themselves today tomorrow and every day because they are not receiving adequate doses of TLC.
When I tell physicians, judges, politicians, advocates, reporters, and Generals that statement have a blank stare, and then they go back to what they were doing => nothing good for humanity.

As an acupuncturist who works with people and animals, there is no such thing as the 'placebo' effect. The animals I treat recover from their paralysis and pain just as well as the people.. Acupuncture rocks.

I assume that you consider yourself to be a 'health practitioner'. If so, your comment is bizarre.

The placebo effect is very well documented, has been known for many decades and is a standard part of the protocol of testing drugs and therapies.

Placebo affect was invented to betray human spirit of those who suffer in pain. Blame the patient. Blame the victim.
That construct is so sickening I want to vomit when I hear it.
There is no such thing as placebo effect only arrogant, ignorant, narrow minded, brain trained, devoid of compassion scientist. This idea was constructed to promote the paradigm of only using drugs and nights to treat physical pain problems. Physical pain problems can be eradicated with physical therapy with and without needling.
Muscles of the first location of most all physical pain complaints. Muscle derive pain is 100% invisible to the human eye and all of technology. There has to be a trust and a bond between patient and physician. This bond was broken 60 years ago.
What do with the people who fail your 2 wrong options for physical pain?
Establish disability.
Blame and shame the those who are suffering in pain.
Call them hypochondriacs, poor historians, up to no good, adversaries, drug seekers, pill seekers, and the like.
Then you construct tort reform so when your surgical procedure fails you have no recourse but to live in your pain alone disrespected.

Yes, placebo is well documented When your goal is to prove that the placebo affect exists.
All you have to do is talk to patients, and they will tell you the truth about their particular medical problem.
If you set of questions which are biased, you can prove just about anything.
Remember the earth was flat at one time?
Remember American slavery?
Remember the American Indians and their plight?
Remember how a guy who wanted to design a superior race which led to 55 million people dying? 20 million starving to death.
Remember Jim Jones?
Remember Charles Manson?
and a distorted, corrupted genocidal scientific community all of who have forgotten the muscle system and all of its therapies. Well, they haven't forgotten it they've just overlooked it because cannibalizing bone and nerves is hugely profitable. Profits will blind most people, and they will do walk past someone who is suffering to get their cheese.

Related but with demonstrable, immediate, and long term results: dry needle therapy.
Acupuncture by another name with less practical knowledge and experience.

I've studied both of them and they both have been watered-down to the point where they can only help maybe 25% of people. This is because the mechanism of action is intramuscular stimulation. If you read C Chan Gunn he will explain it all to you. Thus dry needling, wet needling, bio puncture, prolotherapy - all use a needle thus needle intramuscular stimulation.
The for-profit knowledge Masters don't want you to know that.

Note, this was not verum acupuncture, verum electro-acupuncture was used. Also not clear how the subjects were treated. By the same acupuncturist, or one of several, assigned at random.
For this treatment to work, you need to have a practitioner that is adequately trained in neuroanatomy.

Symptoms of carpal tunnel can also be caused by muscle pain and pressure in the neck and shoulders affecting the same nerves.
2
For this treatment to work you need to be able to access enough treatment for it to be effective. Training in neuro-anatomy, aka where-to-put-the-pins, is implicit in the treatment.

If it's at the neck level, you would get additional symptoms in areas not innervated by the median.
I never had surgery. I have "tune-ups" once every couple of years. It was amazing and easy and inexpensive (especially as compared to surgery.) The other upside was I didn't have any downtime in recovery. I'm a firm believer.
